1/4/2024 EnchantaVaughn Approved (AM) Upon arrival to the Winter Program, client was observed listening and following directions. Client lined up with his peers for special outing to the library. Client exhibited good listening skills and positive social behaviors, as evidenced by him walking quietly with his peers. Client presented himself with a positive attitude and remained on task. (PM) Client was given a break and chance to catch up with his counselor to review new goals and expectations. Client understood his expectations and agreed to go back to class and show his teacher is capable of being a good student. (EOD) Client maintained appropriate social behaviors and was seen following directions until dismissal, meeting objectives 1, 2, and 3, as evidenced by reducing symptoms of ADHD and making healthier decisions.(AM) QMHP welcomed client to school and provided support and encouragement to ensure a good start to his day and to be on track with achieving his goals and objectives. QMHP monitored client individualized behaviors in collaboration with her teacher and other figures of authority. (PM) QMHP reminded client to utilize self calming techniques (i.e. counting and deep breathing) if he starts getting distracted. QMHP brainstormed ways client can make himself better. QMHP isolated client from aggressive behaviors to decrease disruption in class and to reduce excessive talking. QMHP processed behaviors and offered self reflection. (EOD) QMHP taught client alternative method to interpret and cope with stress in the classroom by reevaluating situation or problem in a more positive way. QMHP educated client on coping strategies to deal with his feelings and emotions. QMHP supported client with creating a hobby to engage in during free time and individual breaks (i.e. writing a story or coloring a picture). QMHP challenged client to continue displaying a positive attitude while employing positive reinforcement when client is able to achieve his goals and objectives. QMHP reviewed psycho educational activities to ensure retention and knowledge gain. QMHP provided praise when the client continued to display positive behaviors. QMHP explored client feelings about different subjects in school and how to manage his confidence level and self esteem. (AM) Client greeted QMHP with a hug. Client shook his head in agreement and returned to his daily routine. Client said he woke up feeling good. Client appeared to be ready for today’s activities. Client agreed to try his best to avoid aggressive and impolite behaviors. The client listed many activities in which he wishes to engage as his mental illness symptoms lesson and was given additional feedback in this area. (PM) Client remembered all of the relaxation techniques discussed and smiled. Client assured he would know how to handle his emotions if faced with conflict due to understanding of his recurrent pattern of mental illness symptoms. Client responded well to feedback. Client was acceptive and motivated to make himself better each day. (EOD) Client was able to reflect on his day, admitting to any mistakes. Client said "thank you".
